Your First Visit
What to Expect
We strive to make your dental appointments as pleasant and comfortable as possible. Please arrive 30 minutes prior to your scheduled appointment to fill out any necessary paperwork. You will receive a comprehensive oral examination and x-rays to assess your dental health and plan your personal treatment options. You may also meet with the hygienist to determine the type of cleaning you will need. Our staff will schedule additional appointments for cleanings and other services you may need.
What to Bring
To save time, please bring with you:
- Dental insurance card, if applicable
- Photo ID card
- Completed paperwork. Download the forms by clicking on the links below.
- New Patient Medical History Form
- Consent for Minor Child(Only if you are under 18)
Treatment Planning
Based on the findings of your initial examination, your dentist will develop your personal treatment plan, which will specify the type of cleaning and additional visits you will need and any home care products that are recommended based on your oral health. We will review your treatment plan with you and will never begin treatment without your full understanding and permission.
